A reminder, that Full Payment is due at time of service at Stephens Chiropractic and Wellness.
We understand that life happens! But when scheduled patients fail to show up for their visits, they cause an interruption in the scheduling process, which creates inefficiency in the office. It also creates an unused appointment slot that could have been used for another patient. In order to ensure the best service possible, a 24 hour notice is required for all appointment cancellations or reschedules. Any late arrival more than 10 minutes, without notice, will need to be rescheduled.
No show/No call appointments will be charged 50% of the service fee. This fee will be applied to the account, and must be paid with next service rendered at Stephens Chiropractic and Wellness, or within 30 days.
